Vacterl Association (Fetal Syndromes)

Synonyms: VATER sequence, VATER association, VACTEL syndrome
Definition:Association of malformations including V (vertebral anomalies), A (anal atresia), C (cardiac anomalies), TE (tracheoesophageal fistula or esophageal atresia), R (renal/urinary anomalies), L (limb defect)
What The Referring Physician Needs To Know
  • Patients are considered to have VACTERL association when three or more organ systems are involved
  • Differential diagnosis: Townes syndrome, trisomy 18, trisomy 13, other disorders characterized by vertebral, renal, radial defects
  • Termination of pregnancy can be offered before viability
  • Monthly sonographic monitoring of fetal growth, evaluation of structural defects are recommended
  • Delivery in tertiary center is required for prompt surgical repair and rehabilitation
  • Neonates with VACTERL association should remain on antibiotic prophylaxis for urinary tract infection while awaiting completion of screening
  • Urologic follow-up is crucial since majority of patients eventually require genitourinary intervention
