Tarsometatarsal (Lis Franc) Fracture/Dislocations

Synonyms: Lisfranc fracture
Definition:Tarsometatarsal or Lisfranc injuries may be subtle or complex. Radiographs may demonstrate subluxation or dislocation and fractures.
What The Referring Physician Needs To Know
  • Tarsometatarsal or Lisfranc injuries may be subtle and complex; radiographs may demonstrate subluxation, dislocation or fracture
  • Up to 20% of Lisfranc injuries are overlooked on initial radiographs
  • If radiographs are equivocal, CT should be obtained to exclude subtle mid foot fractures
  • Injuries with no evidence of instability on weight bearing can be managed with closed reduction and cast immobilization
  • Open reduction and internal fixation is sometimes required (Fig. 20, Fig. 21, Fig. 22, Fig. 23)
  • Fusion of fourth and fifth metatarsal bases should be avoided where possible
