Longitudinal sonogram of the right lower quadrant (Fig. 1) demonstrates a tubular structure with internal echoes (star) containing a large echogenic focus (arrows) which shadows (arrowheads). Transverse view (Fig. 2) shows the tubular structure in cross-section, with internal echoes (star) and a diameter of 1.4 cm. Additional images showed the structure to be blind-ending (not shown). Coronal reconstructed images (Fig. 3) from a CT performed on the same patient confirms the tubular structure as the significantly dilated inflamed appendix (star) with surrounding inflammatory changes (arrowheads). Another coronal image (Fig. 4) shows a large density (arrow) representing an appendicolith, corresponding to the echogenic structure seen on sonography.
